The amount of time that experts will take going through this PDF as opposed to paper records will be cut dramatically as each page of every chart will be bookmarked and linked – select a record to view, click on the record title, and experts will be taken directly to where they need to be within each patient chart. Also, in this day and age, many experts would prefer to have electronic medical records to review than 2 tons of paper! Further, the records are organized much like a hospital or a provider chart is organized, so it is much easier and faster for the expert to navigate a pdf in this format, and can save the attorney in costs for expert reviews. Electronic Organization of Medical Records includes the following: (1) Organizing the medical records in chronological order in a pdf for each provider as they are in a hospital or provider chart; (2) Bookmarking the PDF so that all that the attorney or the expert will need to do is click on the bookmark of the medical record(s) they are seeking and they will be immediately brought to the record(s) in the pdf; (3) Bates Labeling the Medical Records in the PDF (if the attorney so desires); (4) Creating a detailed index of the PDF including a description of the medical records (i.e. Discharge Summary, ER Records, Radiology, Labs), the dates of service and the bates range of the records; and (4) Creating an Index of All Providers (so they may see which providers treated the patient from start to finish and when those providers saw the patient) including the Name of the Provider/Facility, The dates of service of each provider and the Bates Range of each provider. I have attached a sample of a very brief provider pdf (with white labeled pages instead of the actual medical records) and a sample of the Index I would provide. This does not, of course, represent an entire chart, but it will give you an indication of what the final product will look like.
