I'm a registered nurse by profession. I have extensive experience in home care scheduling, and patient management. My previous role as a Home Care Scheduler involved utilizing Smartcare and AxisCare to optimize scheduling processes. I effectively managed patient charts, through Electronic Health Records, ensuring accurate data input, and close observation of medications and dosages. My responsibilities included communication with clients, caregivers, and colleagues to resolve any issues that arose, thereby enhancing the overall care experience. I successfully acquired and maintained client and caregiver data, using tools like Jotform, SignWell, and BoldSign. Additionally, I conducted online caregiver training sessions, utilizing Home Care Pulse Training and TelescopeHR, equipping caregivers with essential skills and certifications. To support these training initiatives, I created instructional videos and presentations using Canva. My role also included managing social media communications, particularly on Facebook, to promote our services, and engage with the community. I utilized Trello for data organization, ensuring that critical communications were effectively relayed to my CEO and team members. In terms of financial management, I employed Google Spreadsheets to calculate billing for veterans, ensuring timely submissions to Paradigm Seniors. I reviewed caregiver care notes for accuracy, identifying errors and potential HIPAA violations, and made necessary edits to uphold compliance standards. My efforts contributed to the successful compilation of care notes for state surveys, resulting in our company passing the evaluations.
