the health center or Severe Acute Respiratory illness (SARI) cases admitted at the hospitals to ensure a
completely filled out a Flu Burden of Disease (Flu- BOD) Case Report Form (CRF).
3. Comply with the standard operating procedures in filling out the Case Report Form (CRF).
4. Collect oropharyngeal and nasopharyngeal swabs as required by the Study protocol
5. Keeps a notebook where identification numbers used and assigned are recorded.
6. Comply with the standard operating procedures in the collection, storage and transport of specimen
obtained from ILI and SARI patients from the hospital to central storage and eventual shipment or
transport of specimen to Research Institute for Tropical Medicine (RITM).
7. Review hospital admission logbooks to identify Severe Acute Respiratory Illness (SARI) and nonSARI admissions by gender and age, and record this information on prescribed forms.
8. Submit completed forms promptly and on the appointed time.
9. Makes Quarterly, Semi-annual, Tertiary and Annual reports regarding the results of the research
being undertaken and provides analysis and recommendations (where needed) and submits to Medical
Officer in charge.
10. Bring to the attention of the clinical coordinator progress of work done and all issues, concerns and
