I currently work as a Home Health QA Reviewer, focusing on OASIS review (SOC, ROC, Recert, Transfer, and Discharge), PT/OT Evaluations and Re-evaluations, and skilled visit notes including wound care, PTA/COTA, and HHA documentation. I also prepare Case Conference updates and 60-Day Summary notes to support care coordination and team communication. My goal is always to ensure accurate, compliant, and high-quality documentation that reflects the best possible care for each patient.
