Duties and Responsibilities
Technical Duties
Machine set-up, preparation of dialysate and assembling extracorporeal circuit. Primes the circuit and the dialyzer.
Prepares dialysis machine before the treatment to ensure proper function.
Checks alarms, temperature, conductivity, venous and arterial pressure and air detector.
Carries out hemodialysis prescription efficiently.
Minor troubleshooting of the hemodialysis delivery system.
Patient Care
General nursing care to include proper aseptic technique in Hemodialysis both permanent and temporary access e.g. Arteriovenous Fistula, Arteriovenous Graft or central line catheter e.g. intrajugular, subclavian and femoral catheter.
Pre-initiation check and pre-dialysis patient assessment
Weighing the patient, pre vital signs (blood pressure, pulse rate, respiratory rate, and temperature), evaluation of the patency of the access, signs and symptoms of congestion, bleeding, hypotension, weakness, episodes of nausea and vomiting.
Initiation of adequate dialysis treatment according to physician’s order
Cannulation of Arteriovenous Fistula and Arteriovenous Graft.
Use of temporary and permanent catheters.
Aseptic technique strictly observed
Administration of Anticoagulant as per institutions protocol.
Monitors vital signs and other parameters every 30 minutes.
Attends to patient’s needs during the treatment and observes for any sign of complication that may arise.
Carries out physicians order and administration of medications (oral and parenteral medications) and blood products.
Response to emergency situation
Acts as a team member during cardio-pulmonary resuscitation.
CPR, ambu bagging, administration of emergency drugs and oxygen as per physicians order, vital signs, cardiac monitoring and use of pulse oximeter.
Establish and maintains effective communication with all patience and discipline.
Includes significant others in the patients plan of care.
Termination and reinfusion of blood after completing Hemodialysis without complications.
Post- assessment of patient
Post vital signs, mental status, post weight and evaluation of patient access.
Take responsibilities for direct delivery of patient care by promoting good standard in the quality of care.
Commence or continue treatment schedules as ordered by the physician.
Does health teaching to patients and significant others about nutrition and diet, fluid limitation, signs of congestion, and access care.
Reminds patient about home medications and schedule of next dialysis treatment.
