I am an easily trainable, enthusiastic, flexible but focused and goal-oriented graduate of
Human Anatomy, currently training in IT Support Services, seeking a challenging
position in a reputable organization. I aim to provide humanitarian services,excellent
administrative and virtual assistance through diligence,creativity,adaptability, and
commitment while working as a team player to achieve the organizations vision and
statement of mission as well as to enhance my knowledge about new and emerging
trends leveraging my skills in software tools such as Slack, Loom,Zoom, Trello , Google
Chat, Intercom, Freshdesk, Zendesk, HubSpot, Asana, Calendly, Teams, and SLA
management to enhance organizational efficiency.
WORK EXPERIENCES

Responsibility:
● Provided HTS services across service delivery points at supported facilities using
the approved National testing algorithm.
● Ensured test and start; clients tested positive are provided with comprehensive
HIV services, including AHD screening and linkage to ART and wrap around
services.
● Liaised with all testing points within the facility to ensure all newly positive clients
are linked to treatment.
● Followed up with newly diagnosed HIV-positive index clients to provide Index
Case Testing and Partner Notification Services (PNS).
● Provided Index Case Testing and Partner Notification Services (PNS) for virally
unsuppressed clients.
● Ensured the use of Risk stratification tool for optimum yield of positives within the
facility.
● Received referrals of newly identified and known HIV-positive persons not on
ART as identified in facilities and communities.
● Supported intra-facility linkages by accompanying newly identified clients in the
facility to the point of enrollment and ART initiation.
● Send SMS reminders a day prior to the appointment date of patients scheduled
for clinic visit.
● Called clients who did not keep appointments to reschedule their visits.
● Conducted home visits for clients who aren't able to visit the clinic.
● Assessed and documented barriers keeping clients from enrolling in ART and
provided supportive counselling to help clients identify solutions to the barriers to
ART initiation.
● Ensured tracking processes and outcome is recorded in the contact tracking and
termination form and register.
