1.0
Introduction
This document is written to provide health and safety information about specific project risks known by
the Client, and the contractor (LEAAS) to be applicable to this project. This document also provides
minimum health and safety requirements, standards and expectations that the Lead AI Assemah (LEASS)
must adhere to and ensure a high level of health, safety and environmental performance is achieved and
continually improved.
This document does not alter nor supersedes the Health Safety and Environmental (HSE) requirements of
King Fahd Military Medical Complex (KFMMC) and any provision written shaLL be in accordance with the
standard HSE policy of KFFMC and with highest ethical industry practices.
2.0
Purpose of this document
The purpose of this document is to provide detailed information regarding anticipated site health and
safety matters, and to establish policies and procedures adequate to protect Lead AI Assemah (LEASS)
workers, the public and the environment from the predicted site hazards. This plan is prepared to inform
all field personnel, and our subcontractors (if any), of the potential hazards on the site. However, LEASS as
well as each personnel must assume direct responsibility for its own employees' health and safety. If
applicable, meet recommended all medical surveillance requirements. All site personnel shall follow the
guidelines, rules, and procedures contained in this safety plan.
This document also provides minimum health and safety requirements, standards and expectations that
the LEASS personnel at site must adhere to that all relevant contents are dealt with in a way which is in
compliance with legislation and the ethical concerns for the safeguarding of its employees, the properties
of the client and to any persons affected by the maintenance activities.

Definition of Terms
Agreement: the contractual arrangement between MMO and the Contractor.
Competent Person: means a person who is appointed or designated by the employer to perform specific
duties, which the person is qualified to perform due to their knowledge, training and experience.
Contractor: means an "LEASS" that performs activities or services governed by an Agreement between
LEASS and KFMMC.
CIMR: Contractor Inspection and Maintenance Report-reporting form that contractor will accomplice and

Environmental Aspects - an element of an organization's activities, products or services that can interact
with the environment
Environmental Impacts - are changes in the natural or built environment, resulting directly from an
activity that can have adverse effects on the air, land, water, fish, and wildlife or the inhabitants of the
ecosystem
Environmental Management System (EMS): means an LEASS's Environmental Management System,
which will be used by LEASS to establish environmental policy and to manage the environmental aspects
of its activities, products and services in the project
Environment Plan: means a plan that addresses all environmental procedures necessary to perform the
Services.
Hazardous Material: is defined as any substance (liquids, gases, powders, fibers, chemicals) which can be
inhaled, ingested, or come in contact with the skin or eyes and has the potential to cause injury by way of
its chemical, physical or toxicological properties or has the potential to harm the environment.
Health and Safety Plan (H&S Plan): means a plan that addresses the Contractor's overall Health and
Safety Management System including (but not limited to) project site organizational structure,
competencies, training, risk management, communications, emergency response, etc. to ensure that the
LEASS will achieve an acceptable level of safety performance and complies with all relevant laws, codes of
practice and standards.
Health and Safety Representative - person authorized to management and implement health and safety
programs and procedures
Informative: means the information supplied in general, is a guide only and not mandatory.
IEC: The International Electrotechnical Commission is an international standards organization that
prepares and publishes international standards for all electrical, electronic and related technologies

Safe Work Instruction (SWI): means a set of written instructions, which identify the health and safety
issues that may arise from the tasks that make up a system of work

Standard Operating Procedure (SOP): means a written procedure prescribed for repetitive use as a
practice, in accordance with agreed specifications aimed at obtaining a desired outcome.
5.0
Duties and Responsibilities
5.1
LEASS as Contractor
Provide and demonstrate to the client (KFMMC) a suitable, sufficiently documented and coherent site
specific health and safety plan, based on the client's documented health and safety specifications, which
plan must be applied from the date of commencement of and for the duration of the construction work
and which must be reviewed and updated by the principal contractor as work progresses.
LEASS must take reasonable steps to ensure co-operation between client's representative and LEASS
personnel to comply with HSE Regulations.
5.2
The Site Project Manager
The Deputy Site Project Manager appointed by LEASS) shall be person in charge to implement this HSE
policy and may impose any other procedures or prohibitions believed to be necessary for safe
performance of the maintenance activities. Any other procedures deemed necessary to be imposed shall
be coordinated with KFMMC Engineering Department.

LEASS will develop and implement safe work practices which are generally written methods that define
how tasks are performed while minimizing risks to people, equipment, materials, environment, and
processes. Safe Work Procedures are documented procedures for performing tasks.
These are documented procedures for performing tasks with the purpose is to reduce the risk to health
and safety in the workplace and reduce the likelihood of an injury by ensuring that employees know how
to work safely when carrying out the tasks involved in their jobs. Safe work procedures may also be called
safe work method statements (SWMS).
7.1
Hazard Identification, Risk Assessment and Controls
Upon receiving the project site LEASS shall conduct a Hazard Identification Risk Assessment and
Control which will be a powerful tool to use to ensure workplace safety and reduce risks. It can be
done by systematically identifying hazards, assessing risks, and implementing control measures,
employers can create a safe and secure environment for their employees. This not only reduces
accidents and injuries but also promotes a culture of safety and fosters trust among our workers
and others. It has the basic purpose of Hazard Identification, Risk Assessment, and Implementation
of Control Measures.
Initially, LEASS had determined that following hazard will be present during and anytime that tasks
and jobs are performed in the works associated with diesel generators such as :

Carbon monoxide from a generator's exhaust.
Fires from improperly refueling a generator or inappropriately storing the fuel for a generator.

7.2
Safety Planning

and required to mitigate the recognized hazards and safely complete each activity.

Pre-Task Safety Planning (To be completed by Technical Personnel Foreman). The Technical
personnel foreman or any of LEASS Maintenance personnel will analyze each task to be performed
by scope of work and identify the work sequences, hazards, and controls necessary to protect
workers from the identified hazards. Hierarchy of controls must be observed. The Pre-Task Safety

Plan (PTP) will be communicated daily to each crew performing work on this project. Each
Technician will sign the PTP acknowledging the safety procedures while engaged in the task.
7.3
Safety Inspection
Each LEASS personnel performing work will be responsible for conducting weekly safety inspections of
their work area, tools and equipment (daily). The following inspections will be required as applicable to
ongoing work activities. Safety Inspection forms or Work Permits shall be obtained from KFMMC if the
general procedure is considered.

Weekly Worksite Safety Inspections -Contractor personnel will perform a visual general safety inspection
of their work area where the clients representative. This task is associated in their scope of works in weekly
Diesel Generator Performance Inspections.

Use hearing protection when necessary to reduce noise exposure. h. Ensure the generator is
maintained and serviced regularly, including oil changes, filter replacements, and inspections.
Ensure emergency procedures are in place in case of accidents or incidents.
Personal Protective Equipment (PPE)
In order to ensure that workers wear appropriate PPE, such as gloves, safety glasses, and hearing
protection. LEASS shall by its resources provide all necessary PPE to its personnel assigned in the project.
Other protective equipment shall also be to protect against exhaust fumes, as necessary.
8.0
Emergency Response and Reporting Procedures
LEASS will prepare and submit an emergency action plan (EAP) intended for this project in order to
facilitate and organize its personnel while doing their tasks and when workplace emergencies emergency
situation comes on site. The Emergency Action Plan shall be in conjunction with Emergency Response Plan
of KFMMC.
It is an important condition for LEASS personnel on how to report emergencies. It is important for them to
notify the proper authorities such as fire, medical, or rescue services, as they may also relies on this type of
assistance during an emergency.
LEASS personnel will have a proper training on responding in an emergency situation so that in such, they
will be aware and understand their roles and responsibilities within the plan.
9.0
Safety Training Plans
LEASS personnel for this project shall have a safety, health and environment aspect training and it shall
mandatory for our workers assigned to this project in order to promote and ensure an incident and injury
free environment exists.

Environmental Aspects of Project Performance
LEASS in any ways shall consider its project performance activities shall have a less and minimal impact
with the environment. LEASS will highly focus on the proper disposal of such waste generated from the
project performances. LEASS shall comply with regulations, and enhance our environmental and
sustainability performance.
