POLICIES AND PROCEDURAL SYSTEM ON FILING A LEAVE OF ABSENCE
Vacation Leave
Vacation Leave – is a leave of absence other than sick leave, bereavement leave, and incidental leave. This includes emergency leave of any sort.
1. All personnel must seek approval from Support Manager (SM) for vacation leaves.
2. Notification must be given via email 1 week prior to the date of the vacation leave.
3. If approved, the SM will tally the vacation leave in the Leave Registry.
4. SM will then email the personnel to get their confirmation that it is correct and true.
5. The personnel must acknowledge and confirm the same leave via email.
Sick Leave or Emergency Leave
Sick Leave – is a leave of absence when personnel could not report for work due to illness
Situations in which you can take emergency leave:
• You need to make arrangements because of a death in the family;
• You quickly need to find a babysitter or arrange care for an ill family member;
• You have to find a plumber because your water mains have burst;
• Your partner is giving birth (other short-term absence leave);
1. SM must be notified of the absence first thing in the morning via text
2. This will then be recorded in the General Leave Registry by the SM
3. Ricky must then be notified of the sick leave by the SM.
4. The SM will then tally the sick leave in the Leave Registry.
5. When the Support Admin. Specialist (SAS) returns to work, an email must be sent to the SM of such leave of absence indicating the reason. The CEO Ricky must be CCed in the email.
6. SM will then tally the leave in the Leave Registry
Bereavement Leave
Bereavement Leave – is a leave of absence when a personnel’s family member passes away
1. The SM must be notified of the absence the day itself or the morning after SAS has known about the incident, via text.
2. The General Leave Registry will then be updated on the absence by the SM.
3. The CEO will then be informed of the bereavement leave via email, by SM.
4. SM will then tally the bereavement leave in the Leave Registry.
5. When the personnel return to work an email about the incident must be sent to the SM, CC Ricky.
Incidental Leave
Incidental Leave - is a leave when there is internet outage or electrical outage or computer technical issues.
1. A text notification must be sent to the SM the moment there is internet outage, electrical outage, or computer issue.
2. The SM will then record this in the Leave Registry.
3. When the internet or electrical power resumes and Support Asst. is able to work again, the CS Manager must be notified via cellphone, or Skype.
4. CS Manager will then note in the Leave Registry the time work was resumed
A weekly report on attendance and leaves will be submitted by the CS Manager to the CEO.
