INVESTIGATION REPORT
RE: MISSING HDPE LINER at HIGHHEAD TB14-15
SUBJECT
Sir, please refer to my investigation report regarding the above-subject:

AUTHORITY:
1. Standard Operating Procedure and inherent function as Security Supervisor and Investigator for HedCor MF.

MATTERS FOR INVESTIGATION:
2. A series of incidents involving theft has been reported. The thefts have prompted concerns about infrastructure
integrity, environmental impact and potential disruptions to critical services. This report investigates the incidents,
analyzes finding, and provides recommendations to prevent future thefts and mitigate the associated risks.
III.
FACTS OF THE CASE:
3. This report pertains to the missing HDPE Liner and has been discovered later on 01 July 2024, by Serberus
personnel, Mr. John Fabrigas at Highhead TB14-15.
IV. FINDINGS/DISCUSSIONS
4. Incidents often occur at isolated sections of pipelines or during non-operational hours when surveillance is limited.
5. Thieves typically target sections where HDPE is easily accessible or not adequately secured.
6. The thefts appear to involve individuals with knowledge of pipeline locations and vulnerabilities.

CONCLUSIONS
7. Pipelines in remote or less accessible areas are particularly vulnerable to theft due to limited surveillance and
security measures.
8. Insufficient monitoring during off-hours or non-operational periods contributes to delayed detection of theft
incidents.
9. Collaboration among law enforcement agencies is crucial to identifying and dismantling organized crime networks
involved in pipeline thefts.
10. Implementing advanced CCTV system with real-time capabilities along pipeline routes can improve detection and
response to theft incidents.
VI.
RECOMMENDATION:
11. Increase fencing, lighting, and access controls at pipeline facilities and vulnerable sections.
12. Implement routine inspections of pipeline sections to detect damage or signs of tampering promptly.
13. Foster collaboration. Establish partnerships with local communities, law enforcement, and neighbouring
businesses to share intelligence and coordinate responses to theft incidents.
By implementing robust security measures, fostering collaboration among stakeholders can mitigate the risk associated with
pipeline thefts and safeguard critical infrastructure for sustainable development and public welfare. Continued vigilance and
proactive measures are essential to addressing emerging security threats and ensuring the resilience of pipeline networks
against theft and vandalism.
