WORK EXPERIENCES September2020 - Till date Job Title: TB Surge Linkage Coordinator Ika South Employer: KNCV Nigeria TB Foundation Job Description: I coordinate all TB Surge activities in the cluster, to ensure finding of TB cases and appropriate linkage to treatment in the Cluster I ensure active 100% screening for TB by spokes (Primary and secondary health facilities) I facilitate the identification and complete evaluation (by Gene Xpert and CXR) of all identified presumptive TB in the spokes facilities in the cluster. I ensured that 98% of Presumptive identified during the weekly screening were linked to testing I maintain and manage close communication and coordination between facility screening staff, TB-DOTS, Hub sites, TBLS and SPO. I ensure appropriate documentation of all TB services in appropriate registers at the both the Hub and spokes in the cluster. I ensure electronic entry of data into the project app ( CommCare ) I organize and coordinate monthly meetings at Hub site (when SPO is unavailable) and ensure appropriate feedback on services and referral challenges. I coordinate, collate and transmit weekly TB cascade report from spoke sites I provide oversight and act as data cascade manager for Hub site TB cascade reporting on Commcare and facility registers. Support the documentation of the details of all presumptive cases in the presumptive TB register. Support the documentation of test results in the presumptive TB register. Facilitate the communication of positive results to the patients and ensure all TB patients are started on treatment and documented on the Facility TB register. Collaborate with the facility FAST Coordinators.
