I have worked for 10 years in the mental health field. Through the years I have been responsible for providing case management focused on assisting clients and community members with referrals to other agencies. I've provided assistance and coordinated referrals for medical, social, community, financial, and other needed services. I provide constant follow up with clients, community members and referral agencies to insure referrals are completed. I maintains regular contact with all clients and complete needs assessments and care plans with clients, with a goal of client self-sufficiency; help andempower clients to develop and utilize independent living skills and strategies. I make it a priority to advocate on behalf of clients to decrease service gaps and remove barriers to services. I can use my skills and transfer them to any field and provide services to customers with great efficiency.
