OCCUPATIONAL THERAPY REPORT Name Zayan DOB - DOP - Observation/ Referral: Zayan was Referred By Dr. Arooba {SLP) for his Regular Occupational therapy. Before coming to SACRGRW Zayan was already taking OT and Speech Sessions Assessment Zayan was assessed informally during the session based on the general observation of the therapist and interviewing the parents about his Concerns but the parents are suggested to undergo Formal Assessment as well of Occupational therapy and Sensory Therapy at the age of 3. Report of the Month: Zayan had 7 seven Occupational Therapy Sessions at SACGRW based on improving his Fine motor skills, Gross Motor skills ,Self Help Care skills ,ADLs and Sensorimotor regulation. Zayan has improved a lot in his areas of FM and GM skills as Fine Motor Skills His manual dexterity and power has improved a lot and shows digital pronate grasp but still requires lot more improvement in precision and in hand manipulation skills His right hand is dominant but bilateral coordination is weak and require practice of those activities that involves the usage of both the UL actively Gross Motor Skills Gross Motor skills have improved a lot as he can run and climb but still requires improvement in jumping. His Eye hand coordination is good but eye foot coordination still needs improvement.
