SCOPE OF WORK
The undersigned company offers and agrees, if this proposal is accepted, to enter into
an Agreement with the Department of Community Supervision, State of Georgia in the
form included in the Contract Documents and to complete all work as specified or
indicated in the Contract Documents for the Contract Price and within the Contract
Time indicated in this proposal and in accordance with the Contract Documents.
The firm has examined and familiarized himself with the Instructions to Bidders, all of
the other Bidding Documents, and all of the Contract Documents; that he has examined
the actual area and locality where the Assessment & Planning is to be performed; that
he has familiarized himself with the legal requirements (federal, state, and local laws,
ordinances, rules, and regulations); that he has made such independent investigations
as he deems necessary; and that he has satisfied himself as to all conditions affecting
cost, progress, or performance of the work.
The firm accepts all of the terms and conditions of the Instructions to Bidders. This
company will develop a Comprehensive Facilities Management Assessment which will
include separate phases of work.
The firm aggress to perform the Assessment & Planning in the time specified and
accepts the provisions of and assessment of collection, qualification and presentation
of data and capital plans for approximately 207,769 GSF as defined in the General
Conditions of the Contract for Assessment & Planning.
The firm agrees to walk-through inventorying all interior, exterior, and mechanical
assets for 34 buildings in nine (9) cities performing the duties of:
Site Furniture (benches, trash receptacles, bulletin boards,) and hardscape items
(sign systems, paving materials, and lighting components) should be considered
on an agency‐wide basis and encompass the aesthetic and functional context in
which individual projects will be developed.
Identify any unneeded space resulting from downsizing or reorganization,
including sale of a building.
Identify and train around use of prioritization tools including Building Portfolios,
Investment Criteria, and, for assessed buildings, Project Scoring.
Provide Web-based access to project inventory and summary dashboards.
Training for client-team on how to access and use web-based portal.
Provide copies of all presentations used to review, qualify, and communicate the
needs and plan.
Provide a recommended ten-year project plan by investment criteria that ties to
mission, operations, and finance.
Identify energy savings measures to be implemented or planned.
Discuss preventive maintenance and create a plan and a system to use.
Identify future maintenance costs.
Define current state of repair, life‐cycle status.
The firm will accept two (2) qualification meetings conducted with department experts:
Strategy Qualification meeting to finalize strategy toolkit and capital plan.
Assessment Data Qualification meeting to finalize asset inventory.
SCHEDULE OF PRICING:
Unit cost(s) are to include all direct costs, indirect costs, and profit to perform
the defined work. All unit prices are to be included for future reference in
negotiating a fair price for add/deduct change orders. Change Orders will be
executed in accordance with the General Conditions of the Contract.
